I hope this letter finds you well. I am writing to inform you that after ten years of practicing
as an Internist at K Primary Care and Medical Nutrition Center, I have made the difficult
decision to close my practice. This decision was not made lightly and came after much thought
and consideration.

The last day of my practice will be 8/30/25. Until then, I will continue to provide care
at reduced hours to ensure a smooth transition for all my patients. Your medical records are available
on your portal and can be securely downloaded to your personal computer or sent to your new provider as follows:

• Go to patient portal → My Health → Health Records
• Specify single date of service, custom date range, or click on all time
• Download records to your personal computer or transfer them to your new physician

Effective 8/30/2025, we have retained Cariend, a medical records custodian, to manage the
records for the next 7 years. Cariend is not a medical provider and does not handle billing, medications,
or referrals for the closed practice. Requestors should be prepared to complete the Records Release
Authorization Form and provide a copy of government-issued photo ID, as required by HIPAA. The fastest way to process requests for copies of medical records is to visit Cariend’s website and follow the instructions
posted here: https://www.cariend.com/request-records/

If you do not have access to a computer or internet, Cariend can be reached by phone at 404-991-7080
– please note HIPAA laws do not allow medical records to be released based on a phone or email request.

Request@Cariend.com. However, you should not send sensitive
personal information or request documents through email.

It is important for you to establish care with a new primary care provider by contacting your
insurance company for participating providers.

It has been my privilege and honor to serve as your healthcare provider. I am grateful for the
